Waiver and Health Form

Participation in the Cross & Anchor Summit requires the submission of a waiver and completed health form. These forms require written signatures; electronic signatures are not accepted.

Completed forms can be scanned and uploaded as one PDF document here on the registration form (preferred), or you can send completed forms via email to the Office of Admissions (crossanchor@hcc-nd.edu). Links to the waiver and health form will be included in your confirmation email after submitting this form.
